Admins: How to Import Additional Contacts for Forms

Learn how to send forms that are not student driven.

Important Note: Once a form has been sent, it can no longer be edited.

  1. Select Forms in the left-hand menu

  2. Select the name of the form needing sent (Must be the Other (CSV - Upload) Recipient Type)

  3. Navigate to Send Form

  4. Add a Single Recipient or Download the CSV Template

  5. Complete the CSV Template with Contact Information

    1. First Name

    2. Last Name

    3. Email Address (Must be Unique)

  6. Upload the Completed CSV Template and select Verify Contacts

  7. Each Email will Validate and be marked as Verified or Not Verified

    1. Any Emails that are Not Verified will give error and can be fixed

    2. After fixing errors select Re-Upload CSV

  8. Select Send Form

  9. Enter a Subject (Max 100 Characters)

  10. Enter a Message (Max 500 Characters)

  11. Select Send Form

  12. Form will be sent using: Your Name <donotreply@kudermail.com>
