Educators: How to Import Additional Contacts for Forms

Instructions on how to send forms to contacts outside of parents and student users.

Important Note: Once a form has been sent, it can no longer be edited.

  1. Select Additional Resources in the left-hand menu

  2. Select Form Management from the drop-down

  3. Select the name of the form needing sent

  4. Navigate to Send Form

  5. Download the CSV Template for Other Contacts

  6. Complete the CSV Template with Contact Information

    1. First Name

    2. Last Name

    3. Email Address (Must be Unique)

  7. Upload the Completed CSV Template

  8. Select Submit

  9. Each Email will Validate and be marked as Verified or Not Verified

    1. Any Emails that are Not Verified will give error and can be fixed

    2. After fixing errors select Reupload CSV

  10. Select Send Form

  11. Enter a Subject (Max 100 Characters)

  12. Enter a Message (Max 500 Characters)

  13. Select Send Form

  14. Form will be sent using: Your Name <donotreply@kudermail.com>
